Wound Care Essentials: Wound Assessment (First course released in a series of 10)
Wound assessment is an essential component of wound care management to observe and document current wound status and allow for tracking progress of patients’ wounds.
It should be completed within 8 hours of patient’s admission, throughout the healing process and when new wounds develop. Include wound specifics including etiology, location, measurements, color, exudate, non-viable tissue, odor, and any sign of infection.
Ongoing wound assessment with evaluations will help ensure appropriate treatments, control symptoms, prevent complications, and promote wound healing.
